TCM | Targeted Care Management

 

Brandywine Counseling & Community Services provides statewide community-based Targeted Care Management and peer support services to individuals with behavioral health conditions who need assistance with advocacy, mentoring, applying for entitlements (e.g. Medicaid, SNAP, disability insurance), connecting with community-based treatment providers (e.g. medical, mental health, and substance use), qualifying for other community-based programs, and accessing other support services that are not delivered by their primary behavioral health provider.

The BCCS Targeted Care Management Program also provides assistance to individuals who were discharged from emergency departments, psychiatric hospitals, prisons, and individuals who are experiencing homelessness.

Criteria for Enrollment: 18 years of age or older with behavioral health conditions, including but not limited to, SPMI, SMI, SUD, and co-occurring disorders.

TCM Program Referrals: Providers may make referrals through DTRN or by faxing information to 302.551.2310 (NCC) or 302.688.7200 (Kent and Sussex Counties).
